Single Coordinated Care Plan Project
Milwaukee County Department of Human Services

TMG developed and pilot tested a single coordinated care plan and coordinated care planning process. A major recommendation of the Milwaukee Family Services Coordination Initiative was the creation of a single, coordinated care planning document and process for consumers who are simultaneously involved with multiple service systems, such as the W2 system, the substance abuse treatment system, and the child welfare system. TMG manages this major project involving consumers and representatives of Milwaukee County service systems. Along with facilitating the development of a care plan, TMG has also developed a confidentiality form, a process guide for understanding and working with the care plan, and recommendations for evaluating the project.
Care Management Training and Competency Development Services
Community Living Alliance, Dane County

TMG provides a comprehensive curriculum for care management in an interdisciplinary team based, managed care program using a consumer centered, choice based approach to care. TMG also develops management systems that support the delivery of quality clinical care. These include clinical competencies for medical, nursing and social work staff and an orientation process that assures consistency and teaches best practice. Develops and facilitates a training program to define and improve care management that is consistent with the organization's consumer centered philosophy.
Ethics and Boundaries Training
Long-term Care Case Managers

In keeping with TMG's commitment to offer social workers and case managers training tools that address the issues they face every day, TMG's new ethics and boundaries training was developed specifically for care managers working with long-term care clients. It highlights real life case scenarios, offers three hands-on decision-making tools and uses interactive activities to help case managers roll up their sleeves and tackle complex issues in the long-term care world. The training offers social workers six CEHs toward fulfilling their biannual licensing requirements.
