Integrated Care Program Development and Expansion CCO/CCE is a Milwaukee based non-profit health and long-term care provider. TMG provides CCO/CCE with education and analysis of the environmental changes in the structure, financing and delivery of both health and long-term care services to older adults in the Milwaukee community. TMG assists CCO to identify and select viable program and business development options and plan implementation. Aids CCO in their analysis of the feasibility, planning, and expansion of the Wisconsin Partnership Program into other counties. Assists CCO in their development and implementation of a compliance program to comply with the recommended program guidance of the federal Office of the Inspector General (OIG) for Medicare+Choice organizations.
Integrated Care for Persons with Physical Disabilities-Program Development and Expansion CLA is a subsidiary of a Madison based Independent Living Center offering a fully integrated managed care program to Medicaid/Medicare enrollees. TMG provides support to CLA for building the internal capacity and process to continuously assess market opportunities as a regular management and organizational function, while simultaneously assessing certain targeted current opportunities. Aids CLA in their efforts to raise the awareness and visibility of their organization and continuously review potential collaboration with other community-based providers and service organizations. Assists CLA in their analysis of the feasibility, planning and expansion of the Wisconsin Partnership Program into other counties. Supports CLA in the exploration and development of integrated care pilot programs to new, targeted populations. |
