Case management is at the core of high quality community long-term care services. It is a crucial job that's responsible for balancing, synthesizing, and arranging the supports needed by individuals to achieve quality outcomes in a cost-effective manner. Case management supports individuals to achieve lives of independence and dignity. Lives in which they are actively involved in decision-making. Lives in which they achieve the functional, health-related and personal outcomes they envision.
But developing or maintaining a successful outcome-oriented case management system is complicated. It involves a working knowledge of the delicate relationship that exists between the case manager or care team and the consumer. It is based on a person-centered, strength-based approach.
Fostering a client-focused approach to case management is very challenging-a successful outcome-focused care system can only occur through dedicated efforts at all levels of an organization. More typically, case managers or case teams learn this on the job-from co-workers, supervisors and through working with consumers.
