   OUTCOME AND PERFORMANCE IMPROVEMENT

TMG assists clients to ensure high quality consumer experience and outcomes for individuals, and to maximize the overall performance of long-term care delivery systems.

For the past 20 years TMG has worked in partnership with state level home and community-based waiver programs, long- term care consumers, provider organizations, and local delivery systems on the development and implementation of a quality oversight and management process. This process is called Quality Outcome and Performance Improvement (QOPI).

QOPI is designed to empower organizations and individual users with a focused set of tools and processes to guide continuous improvement in the community-based long-term care environment. This improvement system draws on:

  • A clear definition of program values and goals.
  • Structured measurement of consumer experience in relation to consumer-defined quality domains such as respect and satisfaction.
  • A balanced measurement of system performance across domains.

The TMG approach combines the principles of data-driven quality improvement with the financial and programmatic realities of community-based, long-term care services. It is capable of helping: front line staff (case managers and caseworkers), program managers, and policy makers.
